PPP Sign Up Form

Please read the terms and conditions of the agreement below. Then fill out the form and click "Send". We can set up a dial-up PPP account for you right away. When it is set up, we will fax or email you your account confirmation and instructions for setup.

Note: Cyber Services, Inc. services customers in the Northern Virginia, Southern Maryland, Baltimore Metro- and Washington, D.C. areas.
Terms and Conditions
  1. You may use any alias for your username (8 characters or less, letters and/or numbers) as long as it does not already exist within the list of current users. This alias will also be used to log you into the system.
  2. Cyber Services, Inc. reserves the right to use certain aliases for internal use.
  3. PPP dial-up Internet access is unlimited.
  4. PPP dial-up Internet access provided by Cyber Services, Inc. must be used in accordance with our Acceptable Use Policy.
  5. In the event your user ID is in use more than once at any given time, you will be charged an additional $20.00 for every duplicate login each month in which the activity occurs.
  6. Payment for the above services will be due on the first of each billing month. Payment is acceptable by VISA/MasterCard/American Express/Discover or by check. Long-term contracts pay for the full term of the contract upon initiation, instead of making monthly payments.
  7. Accounts are in default if payment is not received within 30 days after the first of each billing month. If your payment is returned to us unpaid you are immediately in default and subject to a returned check charge of $25.00 from Cyber Services. Accounts unpaid after 60 days may have their service interrupted. Such interruption does not relieve your obligation to pay the monthly charge. Accounts in default are subject to an interest charge of 1.5% per month on the outstanding balance.
  8. Contracts automatically renew for contract length unit. Only a written request to terminate your service relieves you of your obligation to pay the unit account charge.
  9. These Terms and conditions supersede all previous representations, understandings or agreements and shall prevail notwithstanding any variance with terms and conditions of any order submitted. Checking the Agree checkbox at the bottom of this form constitutes acceptance of these Terms and Conditions.
  10. Prices are subject to change without notice. Existing customers will receive 30 days notice.
